docs(claude-code): complete Cleaner terminal handoff

Scope

This is a follow-up to #85, targeting its ry03-cleaner-session-binding branch so the diff contains only 荣耀's remaining Claude Code work.

• Explain the same-terminal interactive Cleaner selection, cancellation, and exact-session resume flow in the Claude Code README and generated /lightrsi-clean skill.
• Assert the generated help text and final release package contents in tests.
• Preserve read-only historical tool-call attribution while a manual Cleaner schedule pauses lifecycle estimation, so the next ordinary request can validate and apply the selected task instead of incorrectly marking it claude_cleaner_current_scope_invalid.
• Add regression coverage for persisted attribution recovery and the scheduled gateway apply path.

Verification

• Claude Code adapter typecheck and build passed locally.
• The full Claude Code adapter suite passed locally: 245 tests, 0 failures (including the release-package smoke test).
• Real unrestricted Claude Code terminal acceptance is still pending after installing the rebuilt package. Do not treat this draft PR as fully accepted until selection, q cancellation, same-session resume, and next-request application are captured in a normal terminal.

No built .tgz archive or machine-specific acceptance files are committed.
