docs(claude-code): complete same-terminal Cleaner handoff guidance

Scope

This is a follow-up to #85, targeting its ry03-cleaner-session-binding branch so the diff contains only 荣耀's remaining Claude Code work.

• Explain the same-terminal interactive Cleaner selection, cancellation, and exact-session resume flow in the Claude Code README and generated /lightrsi-clean skill.
• Assert the generated help text and final release package contents in tests.

Verification

• Skill bridge tests, Cleaner CLI tests, typechecks, and final release package smoke tests passed locally.
• Real unrestricted Claude Code terminal acceptance is still pending. The local Codex sandbox could not inspect the process tree and the test session had no selectable Cleaner tasks. Do not treat this draft PR as fully accepted until selection, q cancellation, same-session resume, and next-request application are verified in a normal terminal.

No built .tgz archive or machine-specific acceptance files are committed.
