Align the VS Code extension with the new patchloom v0.4.0 release.
Changes
patchloom appendcommand.file.appendexample (batch syntax).All local gates passed (compile, unit tests, coverage, package). The managed e2e tests are occasionally slow in this runner but unrelated to these changes.
This mirrors the prior v0.2.0 alignment.
