Release 0.10.4 — the five small fixes merged since 0.10.3 (#175–#179): per-step cost confidence, direct_url.json scrub, wider secret redaction, taxonomy tool names, and .DS_Store ignore.
Version bump (pyproject 0.10.4) + CHANGELOG. DMG preflight: payload changed (src/agentacct, apps/agentacct, packaging) → REBUILD.
