Pass media query breakpoint to column mixins

Allows grid usage for multiple breakpoints:

.col-sm-6 {
  .make-column(6);
}

.col-md-6 {
  .make-column(6, 992px);
}

This approach is backwards compatible for any current uses of these mixins.
