Level up Ghost case workflow and repo polish

Summary

• Add portable case workflow commands: ghost export, ghost import, and ghost delete.
• Add CI, bug/feature issue templates, and PR template to improve contribution quality.
• Replace placeholder screenshots with README-visible CLI screenshots for doctor/list/show.
• Update docs for the SQLite response, self-audit demo, and v2 plan.
• Run Ruff formatting across ghost/tests so CI has a stable baseline.
