docs(changelog): first 0.0.1-alpha release notes - #126

Everything on main is the first alpha (no v* tag was ever pushed). Fold the [Unreleased] block in, add everything merged since 2026-06-10 that the changelog never captured (Send, Swap, the UX foundation overhaul, the MCP sidecar + agent surface + Activity feed, the browser bridge, command palette v2, the chain registry, and the trust hardening: default-deny guardrail, durable daily cap, hardened daemon launch, CSPRNG fail-closed, capability-gated approval, frozen deny vocabulary), and rewrite the stale "Known limitations" (Send / Swap / MCP all shipped). Dated 2026-06-22. release-check.sh passes for v0.0.1-alpha.
