Record started, successful, failed, and halted lifecycle outcomes when an SCM commit resolves an issue. Treat idempotent processing of an already-resolved issue as a halt instead of another success.

Keep terminal outcomes inside the lifecycle context so successful and halted resolutions do not also emit an automatic halt.
Extend the SCM resolution lifecycle to pull-request-based issue resolution so provider health monitoring can distinguish successful, failed, and halted PR resolution attempts.

Extends the SCM resolution lifecycle introduced by the commit-resolution PR to pull-request-based issue resolution, producing provider-scoped started, success, failure, and halted outcomes without adding database state or changing resolution behavior.
