Publish the Claude plugin to a claude-plugin branch - #465

Claude's plugin directory reads a plugin from a fixed path in its repository, here the root. A workflow now publishes plugins/exa to the root of a claude-plugin branch on each change, so the directory can track that branch and see only the plugin. - Add the publish workflow. It commits the plugins/exa tree onto the branch and skips the push when nothing changed. - Replace the SVG icon with a 1024x1024 PNG, the format the directory accepts. The workflow publishes on package version bumps, so merging it alone would not create the branch. It now also runs when the workflow file changes and publishes whenever the branch does not exist yet. - Restore the header comment on why the branch exists and how to publish a plugin-only change. - Describe the version-bump trigger in the README.
