pgcopydb v0.16 (May 22, 2024)

pgcopydb v0.16 is mostly a bugfix release. The main changes internally are
the integration of the Boehm-Demers-Weiser Garbage Collector, and the
continuing work on using SQLite for internal catalogs. The SQLite library is
now vendored-in using the recommended amalgamation source distribution.
