Fix/239 240 duplicate key node range errors

Motivation and Context

• Fixes duplicate key errors for vararg generic type parameters (Illegal state exception (duplicate key) on Apache commons-collections #139)
• Fixes lexical preserving printer error for compilation units that have missing-range nodes (Failure on commons-logging: Range not present on node #140)

How Has This Been Tested?

Added test cases and inputs for both fixes
