I've tweaked the formatting of your Changes file according to the spec defined in CPAN::Changes::Spec.
Following this format means that various tools can more easily process your distribution automatically. You can find out more about this at Brian Cassidy's CPAN::Changes Kwalitee Service (http://changes.cpanhq.org), which is where I saw your module listed :-)
I also added the repository line to you Makefile.PL, so the github repo will appear in the module's metadata. I haven't tested this, as I don't have the prereq modules, but this is the line which will apparently do the job.
