`fastlane/Appfile` is gitignored, so `get_certificates` and each `get_provisioning_profile` call had no Apple ID and prompted for it, four times per `ios release` run. Pass `username:` explicitly, as `latest_testflight_build_number` and `upload_to_app_store` already do, and give the `upload` lane the same credentials as the release upload.
