Adds a Long description for av orphan covering current-branch scope, the trunk-orphans-everything behavior, and the av adopt recovery path.

Orphan branches that are managed by av

Usage:
  av orphan [flags]

Orphan the currently checked-out branch and any child branches that are managed by av.

When run from the trunk branch, this orphans every av-managed branch because all managed
branches descend from trunk. A separate warning for this behavior is tracked in #733.

To manage orphaned branches with av again, re-adopt them one at a time with "av adopt".

The branch-argument feature ask from #720 is intentionally out of scope here. Happy to file a follow-up issue or leave #720 open for tracking that.
