[codex] Refresh README and report preview

Summary

• Refresh the README and README_CN front matter for the lifecycle evaluator, policy, and report preview work.
• Replace the example report fixture with a PR lifecycle report showing selected evaluator pack, policy status, context, and per-evaluator config.
• Update the HTML renderer so summary cards use semantic status colors and Evaluation Context appears after the Evaluation Tree.
• Update the README report preview screenshot to the new rendered report.

Not Done

• No scheduler, GitHub comment posting, or new evaluator runtime behavior beyond the report rendering order/color update.
