[Chore] Temporarily validate mutation failure diagnostics against PR #1494

Temporary production validation only. Do not merge. GitHub refused to reopen #1500 because its deleted branch was recreated, so this is the minimum equivalent replacement and will be closed after evidence capture.

What changed

This temporary branch starts at the unchanged head of PR #1494 (9b2a1b112fe2e598da1e3c29cbcb84f7fbf3eeb2) and applies all three commits from PR #1499 through f0417fa5dd46a37f51a8a2e9294ebfa8d0c1afea.

Why this change was made

The draft exists only to reproduce PR #1494's known mutation failures in GitHub Actions and inspect PR #1499's final failure summary, annotations, reproduction guidance, exclusions, and uploaded HTML report link.

Impact

No code from this pull request should be merged. PRs #1494 and #1499 and their source branches are not modified.
