Problem and behavior

An ADG plugin can remain enabled in the store while its Codex registration is missing. Repeating enable previously did nothing, and an unchanged upstream update could not restore it. Runtime installation failures also lacked a complete command-level result.

Add detailed, read-only Codex inspection and explicit registration recovery:

  • status --json retains existing fields and adds runtimeMissing / runtimeDisabled.
  • enable restores missing or disabled Codex registrations and verifies them.
  • update --repair observes Codex before updating sources, defers its runtime mutations, then refreshes changed enabled plugins and restores missing registrations. Runtime disablement is preserved even when source contents change; ordinary update behavior is unchanged.
  • Recovery follows lock dependency order, blocks dependents of unavailable or failed dependencies, and continues independent plugins. Dependencies outside the selected scope are checked rather than installed.
  • Installation and verification outcomes are distinct. Partial failures propagate through CLI outcomes; unverified installations are not displayed as enabled.

Keep the existing store, lock format, manifest adapters, and runtime CLI boundaries. Inspection is optional for third-party agents. Share dependency traversal between enable/recovery and verification between recovery/sync. No account switching, background service, authentication edits, or production configuration migration is introduced.

Validation

  • Added regression tests first and confirmed the missing recovery / disablement behavior failed before implementation.
  • npm ci, npm run typecheck, npm run build, npm test.
  • npm run test:codex-sandbox, npm run check:docs, npm run check:test-isolation, git diff --check.
  • Isolated real Codex tests cover registration recovery, idempotent enable, runtime-disable preservation, read-only status, and unrelated configuration preservation.
  • Combined tests cover changed sources, failed inspection, enabled-content refresh, dependency failure isolation, and verification output.

Boundaries and review notes

Official Codex account switching is an unconfirmed trigger for registration loss. This PR implements local registration diagnosis and recovery; it does not claim to fix account switching. Existing desktop conversations may need a restart/new conversation to refresh skill discovery.

Explicit directory stores now expose a supported nested marketplace discovery manifest; the integration test exposed the previously swallowed registration error. The temporary-directory cleanup assertion excludes unrelated system-created files while still checking ADG checkout removal.

This is an independent PR against develop. The shared inspection/result contract, recovery orchestration, and CLI propagation form one cohesive behavior change; keeping them together avoids intermediate versions that either mutate before inspection or report unverified recovery as success. Release/CI configuration and lockfiles are unchanged.

Summary by CodeRabbit

  • New Features
    • Added recovery for missing Codex plugin registrations during enable and update operations. Use adg plugins update --repair to restore missing registrations.
    • Status reports distinguish plugins missing from the runtime from those disabled there.
    • Operation results report failures, verified changes, and outcomes that could not be confirmed; command status reflects runtime failures.
  • Bug Fixes
    • Marketplace discovery alone no longer counts as Codex plugin registration; link or sync to register plugins.
    • Codex repair preserves disabled plugins, installs dependencies in order, and verifies results.
  • Documentation
    • Expanded guidance for recovery, account-switch investigations, and privacy-safe troubleshooting.

📝 Walkthrough

Codex plugin operations now inspect runtime registration state and can repair missing registrations. Repair handles dependencies and verifies results. CLI commands report operation failures, verification status, and runtime differences.

Changes: Codex runtime registration and repair

Layer / File(s) Summary
Codex inspection and runtime status
src/agents/codex.ts, src/agents/types.ts, src/commands/status.ts, src/render/plugins.ts, docs/cli-json.md, test/codex-repair.test.ts, test/codex-sandbox.test.mjs
Codex can return a validated, marketplace-scoped plugin inspection. Status reports runtime-missing and runtime-disabled plugins. Documentation and tests describe and check those fields.
Dependency-ordered Codex repair
src/commands/dependency-order.ts, src/commands/runtime-repair.ts, src/commands/state.ts, test/codex-repair.test.ts
Repair orders plugins by dependency, plans actions from inspection state, handles dependency failures, and verifies registration results. Codex enable uses this repair path.
Enable and update repair integration
src/commands/marketplace.ts, src/cli/index.ts, src/cli/handlers.ts, docs/local-plugin-registration.md, test/codex-repair.test.ts, test/codex-sandbox.test.mjs, test/plugins-update.test.ts
adg plugins update --repair inspects Codex before source updates and merges repair results with update results. Repair preserves runtime-disabled plugins. The CLI flag, guidance, and update tests cover this behavior.
Operation failures and command outcomes
src/agents/codex.ts, src/agents/types.ts, src/commands/install.ts, src/commands/link.ts, src/commands/sync.ts, src/cli/handlers.ts, src/render/plugins.ts, src/command-outcome.ts, test/cli.test.ts, test/command-outcome.test.ts, test/codex-repair.test.ts
Agent operation failures and verification details propagate through plugin commands. CLI outcomes and reports distinguish failures, verified results, unverified recovery, and preserved disabled plugins.

Before / after behavior

  • observed — Modified behavior in docs/cli-json.md: Documents that Codex detailed queries may include runtimeMissing and runtimeDisabled as a split of missing, without changing missing; other agents may omit the optional fields.
  • observed — Modified behavior in docs/local-plugin-registration.md: The Codex guidance replaces the claim that Codex reads the marketplace export directly and that link regenerates projections with the statement that Codex loads plugins after CLI registration; it directs users to link for registration and sync for a complete refresh.
  • observed — Modified behavior in docs/local-plugin-registration.md: Adds Codex recovery guidance: status retains missing and adds runtimeMissing and runtimeDisabled; query failures indicate unknown state and queries do not rewrite configuration. Explicit enable restores missing or disabled registrations without reinstalling healthy ones. update --repair updates sources and repairs missing registrations from local snapshots, preserves runtime-disabled plugins, and refreshes changed enabled plugins; ordinary update does not repair intentional unlinks, while sync reconciles desired state. Repair honors component selections and dependencies, reports installation and verification failures individually, and exits nonzero on partial failure without rollback. Other runtimes retain their existing b
  • observed — Modified behavior in docs/local-plugin-registration.md: Adds account-switch investigation instructions to capture scoped status at specified points, compare only ADG plugin/marketplace settings and cache presence, keep backups private, redact shared paths, and exclude credentials, account identifiers, and raw config from telemetry. It states that recovery does not guarantee existing desktop conversations refresh skills and directs verification in a new conversation.

🟡 Changes recommended

The new CLI outcome and summary counting logic can misclassify “unverified install” scenarios and over-count failures, affecting exit codes and reported totals.

What changed in this PR

This PR improves Codex runtime reliability by adding read-only inspection of Codex plugin state and introducing explicit repair flows to recover missing registrations while preserving intentional runtime disablement.

Changes:

  • Add Codex “detailed inspection” (inspectPlugins) and surface runtimeMissing / runtimeDisabled in status (including --json).
  • Implement scoped Codex registration repair (dependency-ordered, verification-aware) used by enable and update --repair, plus verification wiring in sync.
  • Improve runtime operation reporting (failures, unverified installs) and add regression/integration tests + documentation updates.
