Flash-Next NVFP4 with TP4/EP4, attention DP, MTP3, and PLE host offload. The
Qwen3.8 test covers 2,048- and 8,192-token graph buckets, including native QSA
sparse-attention dispatch for a long prompt; the other tests cover both
context-only and mixed context/decode batches using the KV cache.

The following restrictions are enforced:

- `torch_compile_config`, LoRA, and multimodal models are rejected during
engine initialization.
- `torch_compile_config` and LoRA are rejected during engine initialization.
Multimodal wrappers are also rejected unless the multimodal encoder is
explicitly disabled and the wrapper exposes a decoder-only text model.
- Speculative decoding is supported.
- Context-logit requests run eagerly instead of replaying a breakable CUDA
graph.

This guard can never fire. _attn_impl forwards output_sf=None, and the TRTLLM backend only allocates output_sf via create_output when forward_args.output is None (trtllm.py:1829). Because output is caller-provided here, output_sf stays None regardless of quantization settings.

The real hazard is the other way around: when attention._use_quantize_output() is true, _attn_impl still sets out_scale (attention.py:888-890), so the kernel is told to quantize while writing into the bf16 buffer allocated from q.new_empty in [module.py:484](https://github.com/NVIDIA/TensorRT-LLM/pull/19330/files#diff-85c9ac012e0b5a9e4a3ad4935c475daca925563cf17314f6d4aacaf2894521a6R484). Qwen4-Exp is safe only because attn_output_gate=True disables output quantization (attention.py:789).

Suggest either (a) raising up front on attention._use_quantize_output() with a message that says the QSA bridge requires an unquantized attention output, or (b) allocating the buffers via create_attn_outputs like Attention.forward_impl does so the NVFP4-output path works. The current message ("output gating requires...") is also raised regardless of whether output_gate is set.

Add coverage for the quantized-output rejection.

No supplied test makes _attn_impl return a non-None output_sf. A regression could remove this error and let the bridge discard required output scale data.

Add a case to tests/unittest/_torch/modeling/test_qsa_runtime_wiring.py. Return a scale tensor from the dense stub and assert the exact RuntimeError.

As per path instructions, a changed error path requires a meaningful test.

Test MRoPE state through the fixed-output bridge.

The current bridge tests pass mrope_config=None. They do not detect dropped or swapped mrope_rotary_cos_sin and mrope_position_deltas.

Pass distinct sentinel tensors through test_qsa_fixed_output_bridge_captures_index_projection. Assert that the dense bridge receives each tensor in the correct argument position.

As per path instructions, materially changed runtime behavior requires meaningful coverage.

The comment describes the q.shape[0] > num_tokens clause as covering BCG warmup, but it is true for any padded prefill, including the piecewise/torch.compile path (model_engine.py:4318 sets padded_num_tokens there too). That path previously sliced in-line and now goes through the custom op and a full-bucket output.zero_() + copy. Probably fine, but worth stating in the comment and confirming the piecewise QSA path was exercised, since the E2E only covers BCG.

Could we extract a QSA core function and call it directly from both the normal path and the eager bridge, instead of re-entering QSASparseHooks.forward() and using index_projection is None as a re-entry guard? needs_fixed_output currently combines BCG dispatch with padded warmup (q.shape[0] > num_tokens), while index_projection combines data reuse with control flow. Padding should remain supported, but it should not implicitly select the BCG bridge. Separating projection, live cache/dense-sparse dispatch, and output padding would make the BCG boundary explicit.

# Keep the token-wise index projection captured. Cache mutation and
# live batch dispatch execute in the eager bridge below. The same
# bridge preserves physical padding during graph warmup, which runs
# before capture and must still return a bucket-sized output.

Test cache updates before the dense-threshold fallback.

The supplied below-threshold test uses project_and_update_cache. It does not exercise the new supplied-projection branch.

Add a direct QSASparseHooks.forward case with qsa_index_projection and a below-threshold KV length. Assert that update_cache_and_compress receives only active token rows before forward returns None. Without this invariant, a later sparse dispatch can read an incomplete index-cache prefix.

As per path instructions, changed fallback and cache behavior requires meaningful coverage.
