Replaces the single hardcoded SELF_HOST_URL self-host maintenance workflow with a discover -> maintenance matrix: a discover job parses the repo variable SELF_HOST_ENVIRONMENTS (comma-separated) into a JSON list, and a maintenance job fans out strategy.matrix.environment over it with a per-job environment: context, so each self-host instance supplies its own SELF_HOST_URL/INTERNAL_JOB_TOKEN via a GitHub environment while every job reads the same secret name. Falls back to a no-op (skipped) run when SELF_HOST_ENVIRONMENTS is unset, so a single-instance or not-yet-configured operator sees no behavior change. Closes #4899
