fix: harden release packaging and unsafe autofix paths

• stop unsafe generated test-method and decompose writes from auto-applying in refactor write mode
• harden release package parsing to accept extension command output consistently and parse artifact payloads robustly
• block releases from non-default branches to avoid prod drifting ahead of main
