Full write-up of the GitHub Actions workflow audit covering all 5 changes made across the fix/* branches, plus 2 issues that need maintainer discussion before any code change.

README explains every change in plain language: what was wrong, what was changed, and why it matters. Issue files in issues/ are standalone write-ups suitable for upstream contribution discussions.

Updated the condition for the thank-you job to check for true explicitly and added a reference to the base SHA when checking out the repository.
