Steps to reproduce

After manual edits to document chunk content, the text in the database is updated, but the vector database still retains old vectors. This causes inconsistencies between search results and actual chunk content. Users may experience:
Updated content displayed on the page
Old content still being matched in Q&A / retrieval
Vector status may also become inconsistent when enabling or disabling chunks
