Support an initial fee (or free), and then a subsequent regular fee

Some folks want to charge less for the first month, or nothing, or more (a setup fee, etc.).

I know for sure at least PayPal and Authorize.net support this.  Amazon would as well.  Stripe probably does. We should support this.
