Add confirmation to gear request form

"This isn't urgent since we've already crossed this bridge for 2019, but for the future, it would be super helpful if the gear request form on the database had a confirmation screen when people hit "submit."  We've been getting a lot of people who try to resubmit and get an error message and assume it didn't go through at all.  "
