Add editable subject field for email templates

Currently, the email subject lines for email templates in the commission plugin are empty and not-editable.
Editors are unable to set an email subject for these templates. OECS has requested we include a subject line for their emails, but having this be something they can do themselves would be good. It is already the case for the regular email templates that live on JW main.
