Usability Updates

UIowa requested the following:

• Make affiliations not required
• Make page numbers more flexible for chapters (allow range)
• For an edited volume add the option to mark a contributor as an editor and display only the editors for edited vols

Non dev:

• Load the oa logo
• Upgrade to latest plugin version
