Clicking options in the formatting toolbar attempts to close the editor

When creating problem blocks within a library, clicking certain options on the format toolbar attempts to close the problem creation dialog.

I found a similar bug, possibly related:

1. Open a problem component and make some changes
2. Click on the grey background or press Escape, and you'll be asked "Exit the editor?". Press Cancel to keep the editor open.
3. Select some text in the answer boxes.
4. Now the Bold/Italic/etc. buttons are not clickable at all (they do nothing).
