?v6.0.0?: Remove non-ignore TKR mpi module

@hppritcha and I discussed it at lunch today: remove the (old) non-ignore TKR mpi module. It is only for gfortran < v4.9.2.

This will basically:

• rm -rf ompi/mpi/fortran/use-mpi-tkr
• remove a bunch of associated configury
• edit README to remove all mention of the TKR-based mpi module

After discussion at the Dallas Feb 2016 meeting, we decided to punt this to v3.0.0 (we have to do it at an x.0.0 boundary because this will affect our definition of backwards compatibility). It seems too early to remove this -- e.g., RHEL 6.x gfortran is v4.4.7.

this is definitely too early for RHEL 6 series
note RHEL 7.x fortran is v4.8 (which is less than 4.9.2) and does not support IGNORE_TKR too
(and i have no idea about the RHEL 8 roadmap, nor OpenMPI 3)

I'm happy with seeing stuff go away, but my observations have been that
installations are very slow at moving away from old compiler versions.
