Force redirect to home page from /find-representative if the user is coming from anywhere other than the "Add Action" page

...otherwise, the user can select a rep, and then add the activity to the list, but the activity itself never got added, so you'll have an activity in the list with no data aside from the list of reps.

There should be some check for a property that you would only get if you just clicked on the "Add Action" button from the Add Action page.
