[Feature] Annotations on Monitor Grafana Dashboard #319

In order to place the dashboard into a specific Grafana folder, an annotation is required. It would be nice if the annotations on the dashboard configmap could be specified in the helm values.

@ikogan Just created the PR #322 following this suggestion. Note that its also needed to configure the grafana sidecar provider in order to get the dashboard into the desired location.
