Copilot Free: issues with quota not renewing or wrongly applied

We are receiving reports from users on Copilot Free that see their quotas not being refreshed, even though the date to refresh indicates it should have happened.

This is a meta issue collecting all the users impacted:

• I've been interrupted, but the free count hasn't reset #4180
• "Completions limit reached” warning doesn’t reset vscode#237550
