release: v1.2.0 — Tool-Boundary Guardrails Complete & Structured Synthesis Follow-ups (#171)

Bumps version 1.1.0 -> 1.2.0 and rolls the [Unreleased] changelog into a
dated [1.2.0] section (both CHANGELOG.md and docs/CHANGELOG.md).

Contents already merged via #168 (tool_results guardrails, closes #165),
#169 (conditional/single-pass structured synthesis + structured_status,
closes #164 and #166), and #170 (guardrail observability events, closes
#167). All additive; frozen 1.0 API unchanged.
