add `index_block_hash` and `tenure_height` to transaction responses

Transaction responses such as https://api.hiro.so/extended/v1/tx/0x02dc734047db7989b75b3c732f2f74a2682f36688d205493c35bb7db902a6b74 do not currently return values for index_block_hash or tenure_height. These values should be added to all responses that return a transaction resource.

index_block_hash is a column in the txs table, but tenure_height is not. In that case, the tenure height could be pulled with a simple subquery like (SELECT tenure_height FROM blocks WHERE index_block_hash = txs.index_block_hash) AS tenure_height
