NOP not working

I've now deployed the app in 3 different ways - npm, docker and k8s - and NOP doesn't work in any of them, it just posts a comment that there are no changes. Otherwise the app works fine it makes the changes, reverts manual changes and so on. I couldn't find any logs related to this. Does anyone have any ideas what might be the issue? According to the docs this feature should be functional.
