On my fresh Manjaro XFCE install, monpad "just works" with all the Steam games I've tried, without setting the environment variable. This is surprising, but excellent, news.

We should investigate what the determining factor might be, and which distributions are affected. Previous tests were on a 3/4 year old Mint LTS.

On Overcooked 2, I do need the environment variable...

There seems to be no harm using the crowded layout's mapping for the default layout, seeing as the buttons are a superset. Although I have seen some strange one-off failures like BtnMode mapping to right-click - this only seems to happen when starting Steam and is solved by starting it again.
