Root cause identified and fixed in v1.40.7 (commit 307d3f1, just deployed).

What was actually broken

`MediaBulk`'s "delete" action (the bulk delete used by the checkbox + Delete button at the top of the media library) was running a direct DELETE on `media_items` without first cleaning up the foreign-key references the way the single-row delete already does. Any track that had ever been added to a playlist (which on Anti Media is most of them) tripped this constraint:

The handler logged the error and returned HTTP 500 "Operation failed". HTMX, by default, does not swap on 5xx — so the click looked like it did literally nothing. From the prod logs I can see your six attempts today (11:31, 11:32, 12:06, 14:54, 14:55) all hit the same FK error.

The fix

`MediaBulk` "delete" now wraps the whole thing in a transaction and calls `adminDeleteMediaReferences` first, mirroring the single-row delete path. Playlist items, schedule entries, mount-playout state, play-history references, clock slots, and underwriting obligations are all cleaned up before the media row itself is removed. Includes a regression test (`TestMediaBulk_Delete_RemovesPlaylistReferences`) so this can't silently come back.

v1.40.7 is live on prod. Try the bulk delete on Anti Media again — both the top "Delete Selected" button and the per-row trash. They should work now.

If they STILL don't work after a hard refresh (`Ctrl+Shift+R` to clear the old JS bundle), open the browser DevTools → Network tab → click delete → capture the request row (method, URL, status code). That'll tell me in 5 seconds whether the new code is being hit and what's failing.
