Basic example doesn't work - JS error

Hi,

Trying to load the basic example for Form Builder but it doesn't load and starts with a JS error.

Wonder if samples are outdated or I'm missing somthing?

Copied the code here from Github, and also used another sample from JS Fiddle https://jsfiddle.net/travistidwell/Lq9vgka4/ and for both the page loads with same errors.
