The NIHSS calculator implemented here is a simple reproduction of a publicly available clinical scale published by the National Institutes of Health. It performs arithmetic on user-entered numbers and outputs text. It does not interpret imaging, analyze patient data, or make treatment recommendations. The clinician using it is responsible for every score, every piece of generated text, and every downstream clinical decision.
If you are a licensed clinician using this tool:
- You are responsible for verifying every score and every generated piece of documentation against the patient in front of you, before anything is entered into a medical record.
- This tool does not replace clinical judgement. Templates are starting points, not finished notes. They must be edited to reflect the actual patient.
- Cross-reference against current guidelines. Templates in this repository reflect a point-in-time authoring and may become out-of-date as guidelines evolve (AHA/ASA, AAN, NICE, etc.).
- Do not enter Protected Health Information (PHI) into this application. The application is static and does not transmit data to a server, but users should still avoid pasting patient identifiers into any form field in keeping with good PHI hygiene.
