I placed the headset into the jack and to my surprise it fell right off into the unit. Being a radio tech, I opened the unit up and to my surprise if looked like cold solder joints. When I placed it under the microscope, I noticed that one tab had completely broken off. So that may have been the only one holding it. The design of that jack is flawed due to the pressure that is used when pushing a headset jack in.
You should change the jack to one that mounts into the board, rather than one being just placed on top with no real anchorage.

I also should add that the board no longer functions correctly. When the listen is ticked, the signals freeze in place. Only when the listen is unchecked does the tiny SA ultra come back alive.
