Note with heading formatting is understated

We've used notes with headings to give the content more visual importance. In v3, the custom heading and text box are greyed out.

Steps to reproduce

1. Build the logstash.md doc set
2. Go to http://localhost:3000/introduction.html

These were meant more to replace blocks such as these:

You should be able to add a custom tile to a note:

However the heading will always include Note: as prefix.
