docs: reconcile secondary docs to v4 numbering/phases#21

Propagate the v4 recut through the rest of the repo so nothing still references the old numbering/tier names. Renumber factor refs by name across docs/ (00-SUMMARY, README, principles/*, reference/*, explanation/*, how-to, getting-started, tutorials), CONTRIBUTING, GOALS.yaml, and issue templates; rebuild factor tables to v4 order/phases (Prepare/Bound/Select/Govern), insert IV Enforce Least Privilege, fold Harvest Failures into X Compound Knowledge, rename Measure What Matters -> Measure Outcomes. Heroku crosswalks (comparison-table, evolution-of-12-factor) re-aligned with a conceptual-lineage note. SVG diagram name-text swapped (Measure Outcomes / Compound Knowledge); diagram numerals/layout are a known follow-up (assets, not deployed). All factor links resolve; structure + stale-framing gates pass.

Follow-up to v4 (#20): propagates the new numbering/phases through every secondary doc so the repo is internally consistent — no doc still says 'IV. Research' or 'Harvest Failures as Wisdom' or 'Foundation/Flow/Knowledge/Scale'.
All factor links resolve; structure + stale-framing gates pass.
