Constraint panel behavior

I am observing some odd behavior on the Constraint window.

1. From empty constraint panel
2. click "Add"
3. a (the) panel opens in isolation mode with a "1" bullet in the header indicating this is an array panel, all elements are visible, but cannot cancel isolation mode, and this is not an array.
4. click "OK"
5. click "Delete"
6. click "Add"
7. a (the) panel opens in isolation mode with a "1" bullet, but NO elements are visible.

I also had the "Add" button turn to "OK" once, but can't find the path to reproduce that one again.
