In some older versions, quick swapping a helmet on while wearing a hat makes it so you can hold the hat item. This item can then be placed in item frames. However, upon clicking the item to equip or move it, it should disappear.
On some versions, when equipping a hat, you might end up with a phantom duplicate of that hat in your inventory. This phantom item disappears once you access a crafting table, or try to click it.
On creative mode the hat items can be moved freely, but not placed again in the helmet slot. Once the game mode is changed to survival, they disappear once clicked.
Having the server resource pack enabled is necessary for hats to be properly displayed. Optifine is also known to be problematic.
This is likely a problem with the model .json not referencing the texture correctly.
If you're playing on 1.19.3+, this could also be caused by a lack of a texture atlas, or a poorly configured blocks.json file.
Due to the file format, options.yml is quite finicky.
You're probably missing some indents before the second hat ID, therefore the parser considers all that data part of the first hat.
