The triangle shape in adapted single-region meshes appears to be wrong. Changing adapt_smooth has no effect in at least one case. See directory /scratch/gpfs/akleiner/single-region-adaptation on stellar. In this case the triangles appear very uniform around the last-closed flux-surface.
