[enhancement]: Add separate page with Netlify form to request a new resource #46

What
Add a secondary page linked somewhere near the top for requesting a new resource. Otherwise, link the "About" section at the top of the page and include a link there. Maybe rename "About" to "Contribute" or something like that so it's clearer for someone searching the top of the page.
Why
We need an easy way for people to submit potential new resources that also fits the required set of data. This makes it so people can request new resources without having to make a GitHub account to open a new issue.
