Ensure All Save/Update Operations do not call `.refresh(Refresh.True)`

Forcing a refresh is not good for performance, only when needed should we look to utilize the `/util/sync` to trigger this functionality as needed.  We should not force this operation in Elastic, it already is configured at an index level for how often it should refresh.
