Add device identity attestation for patient vitals ingestion

Problem: Device submission currently relies on patient auth and loose device linkage.
How to implement: Register device signer identity, bind it to patient and calibration lifecycle, require device signature for device-origin readings.
Done when: Spoofed device submissions are rejected.
