This looks very likely to be the same permission-flow issue we recently fixed in main: concurrent permission modals could overwrite each other, and earlier pending requests would eventually time out and surface as "Permission denied" for tools like bash / write_file. We also fixed the backend-side permission response handling so approvals no longer get stuck waiting in the queue.
Relevant fixes now in main:

• 9a35860 (merge of fix(ui): serialise concurrent permission modals with a lock #66): serialise concurrent permission modals with a lock
• 69c85e4: resolve permission/question responses immediately instead of routing them back through the main request loop
  Could you please retry on the latest main (or the next release) and see if the problem still reproduces? If it does, please share the exact prompt / tool sequence (for example, whether bash and write_file were requested in the same assistant turn) plus any logs or screenshots. That would help confirm whether there is still a separate bug here.

I pushed another follow-up on top of the earlier permission-flow fixes.

What changed in main now:

• when a mutating tool is denied after the confirmation prompt, the tool result no longer falls back to a generic Permission denied for bash
• it now returns the actionable permission reason instead, including the /permissions full_auto hint

Focused validation:

• PYTHONPATH=src uv run pytest -q tests/test_engine/test_query_engine.py tests/test_prompts/test_claudemd.py tests/test_commands/test_registry.py
• 43 passed

Given the earlier queue/confirmation fixes plus the clearer denial path now in main, I’m closing this one.
