Problem

New issues currently have inconsistent structure, which slows triage.

Scope

• Add issue templates for bug/feature/security docs
• Include reproducibility and acceptance fields
• Add guidance for logs/screenshots without secret leaks

Acceptance Criteria

• New issues open with useful structure by default
• Triage quality improves (fewer clarification cycles)
• Templates align with repo security policy
