docs(readme): upgrade badges to for-the-badge style and add distribution links
- Replace flat-square badges with for-the-badge style across README files - Add Open VSX, GitHub stars, CI status, and last commit badges - Update MCP version badge to show 2025-11-25 spec - Add direct links to Glama, PulseMCP, and MCP.so listings - Remove generic Smithery link - Organize badges into centered groups (versions, stats, status)
